NEW ENROLLMENT PROCEDURES INFO involving

 

INITIAL and SF86 FORMS:

 

Enrollment/Operational Support (OS):

Regarding the following items required as part of the Enrollment process for new members:

Please note the changes described below:

These items are needed:

*       ANSC-7001f Enrollment Application – Updated 01/2007 (Click to Link)

*       Two FD-258 Finger Print Cards

*       ID Request Form (Click on link)

        Credit Report Form (Click to link)

*     Digital Photo of prospective member in CG Tropical Blue Long (light blue shirt)        standing in front of red background.  Please save photo as member’s name; i.e.doe.jane.jpg

 Enrollment applications and Finger Print cards that have been hole-punched or stapled will not be accepted. 

All packages must be on single sheet page; packages can not be printed front to back (2-Sided).

Application needs Flotilla Certification, “Flotilla Commander Signature”, the Flotilla Commander must sign and date application.  Other names/signatures will not be accepted and package will be returned to Flotilla for correction.

        Please do not submit Enrollment/PSI packages to DIRAUX without all required documents.

Enrollment/Direct Operational (DO):

          Only sections I-IX needed (pg. 1-3)

*       Two FD-258 Finger Print Cards

*      DHS 1100-9 – DISCLOSURE AND AUTHORIZATION PERTAINING TO CONSUMER  REPORTS  PURSUANT TO THE FAIR CREDIT REPORTING ACT

*       SF-86 National Security Questionnaire – E-QIP submission. (Click on link) 

*       PLEASE SEE E-QIP SUBMISSION EXPLANATION BELOW

*       ID Request Form (link at top)

*       Digital Photo of prospective member in CG Tropical Blue Long (light blue shirt) standing in front of red background. Shot needs to be from mid chest to a few inches above top of head. Please save photo as prospective member’s name; i.e. doe.jane.jpg.  This is submitted by e-mail.

E-QIP SF-86 Submission:

SF-86 National Security Questionnaire is now processed at the Office of Personnel Management (OPM) web site called E-QIP. 

Flotilla Commander will need to request SF-86 for prospective or existing member (applicant) through the DIRAUX PSI Coordinator Mr. Luke Bailey. 

Applicant will receive e-mail with link to web-site from Mr. Bailey, they will log in by SSN and enter three golden answers which pertain to them directly (this info is obtained from their ANSC-7001).  Applicant has 30 days from the date they receive the e-mail to initiate and complete form.  Applicant will be able to add data and save, allowing them to complete form at their convenience within the 30 day window.

Upon completion of SF-86, applicant will be prompted to print three pages:

*       Signature Forms

*       Authorization for Release of Information

*       Authorization for Release of Medical Information (if applicable)

        

These forms will need to be signed and all originals will need to be mailed to the DIRAUX Office. After printing three pages, applicant will then click on “release to reviewing agency” button.  Package can not be processed until form is released by applicant and original forms with signatures are received by DIRAUX Office.
